When in the multiplayer lobby, you’ll automatically download the mods of the host - I presume this is difficult to set up for hotseat which is why we can only pick the DLC list?
Yeah, unfortunately I think that’s way out of scope for PYDT 
Would it work if the mods are manually installed (with the same version etc.)?
Yeah, I think others have started games like this.
